Cancellations and Refunds
We are aware that emergencies and demands for family or business can occasionally arise. Due to an apparently reserved position, however, failing to call and reschedule or cancel your appointment prevents other clients from attending their own appointment.
All cancellation and refund requests must be made by you or the credit card holder and must include the name of the user and/or transaction number.
Refunds will be credited back to the original credit card used for payment within 30 days from the date of receiving the cancellation request.
Cancellation Policy
Any service/treatment/program cancelled within 4 –hours of the scheduled appointment will result in a 50% charge of the appointment fee to the credit card stored on file.
Failing to attend or notify us of failing to attend (No Show) your appointment will result in the full amount being charged to the credit card on file.
The Client agrees that failure/refusal to sign the informed consent form will result in a charge of 50% of the appointment reservation fee.
Cancellation Policy – Group Bookings
Group cancellations within 4-hours of the scheduled appointment will be charged 50% of the cost for each persons reserved appointments to the credit card(s) on file.
Group No-shows will be charged at full price for each person for all reserved appointments to the credit card(s) on file.
Cancellation Policy – Package Deals
Package services are strictly non-refundable and non-transferable from the moment they are purchased. However, the amount paid may be exchanged for other services or products subject to the discretion of Healthcarebia Home Health Care Center.
Late Start Due to Client’s Circumstances
We are aware that emergencies and demands for family or business can occasionally arise however we aim to keep our services to schedule. If you are unable to start your designated service for more than fifteen minutes past your scheduled appointment time, we may not be able to serve you, though we will try to accommodate you when possible. If we are unable to reschedule you, a 50% late cancellation fee will apply. (Example: Client was not prepared for visit, busy with children or other daily activities.)
Cancellation Policy – Appointment Abandoned
If an appointment is abandoned, the client will be charged the full price of the reserved appointment price. This fee applies in the following circumstances, including but not limited to:
-Nurse dispatched but client unable to receive service because information was not disclosed at the time of booking (example: alcohol withdrawal, drug abuse)
-Nurse dispatched but client unable to receive service because customers observations/vitals signs are out of range
-Nurse dispatched, service not completed due to the client’s physical or mental condition (example: could not tolerate it, Vasovagaled and did not want to continue)
-Nurse dispatched but service not completed because of time constraints
-Unsafe or unsanitary environment
This fee will also apply should the client facilitate a hostile or unsafe environment as perceived by the nurse during the visit. This provision does not apply if the session proceeds normally in which case the client will only be liable for the normal cost of the service.
In case of IV therapy - This fee will NOT apply if:
-Nurse dispatched but unable to start IV after 3 (three) attempts. Note: The nurse must be allowed to attempt the placement at least thrice. If the client chooses not to continue after the first or second attempts, then 50% of the fee will apply.
Cancellation Policy – Nurse Dispatched and En Route
If a nurse has already been dispatched to the client’s location, or if you are not present at the given address within 10 minutes of the nurse’s arrival, you will be charged in full to the held card on file.
Pre-payment and Package Period of Validity
Any purchases whether in part or in full made towards any product or service offered by Healthcarebia Home Health Care Center are hereby void if not scheduled and completed within Two months (2 Months). If the service is not scheduled and completed within that time frame, any payments made would not be considered valid, and are 100% non-refundable. The paying consumer accepts this condition upon release of any payment to Healthcarebia Home Health Care Center LLC.
